Build a form

1. Choose New form.
2. Name the form. The trip or event and what parents should know (dates, cost, what to pack, who to call) are optional.
3. Every form already asks for the parent's name, email and the student's name. Add your own questions: Short answer, Long answer, Checkbox, Date or Signature. A parent or guardian signature is included to start.
4. Mark the questions that are Required, then choose Create form.

Send it to parents

Open the form and choose Copy link to text or email it, Share it from your phone, or Print the QR code for a flyer.

What parents see

1. They open the link in their browser. No account and no app.
2. They type their name, email and their student's first and last name, then answer your questions.
3. If there's a signature, they sign in the box with a finger or mouse, then choose Sign and submit.

The page tells parents their answers go only to your youth team. Fill it out for another student lets them send one for a sibling.

See who signed

The form shows how many have signed. Each response lists the parent, their email and their answers, and whether the student matches your roster.

Export CSV downloads every response. Open parent page shows you what parents see.

Close the form

When the deadline passes, open the more actions menu and choose Stop taking responses. Parents who open the link then see that the form is closed. Reopen form turns it back on.
